This newsletter will be our way of communicating surgery updates more regularly with everyone to keep you more informed.

What we plan to do is to keep you updated with any updates and news here within the surgery. We also want to celebrate our successes with you. We also appreciate your input so if you have anything that you would like to say, please come and speak with a member of staff!

World Health Day 2024

World Health day is internationally on April 7th. The day is sponsored by the World Health Organisation (WHO), and every year a different global health issue is highlighted. The theme for World Health Day 2024 is ‘My health, my right’. This years theme was chosen to champion the right of everyone, everywhere to have access to quality health services, education and information.

Solent connects- Meeting people where they are at Vision

Our ANP Naomi is currently involved in the ‘Solent Connects’ project. Here is what the project is about and its aims…

Our vision is to make meaningful connections with the local community that are both beneficial and sustainable. Recognising the huge amount of work that the social prescribing team does with individuals, this project aims at reaching the many; to raise the profile of Solent in the community; to focus on engagement with groups and neighbourhoods; and to, reach out, meeting people where they are at in order to proactively address their health care needs.

The NHS Long Term Plan (NHS England, 2019) outlines that partnership working in the community is an essential component in the provision of primary care in the future. Linking NHS provision with community support groups, charities and agencies not only provides a more cohesive service but aligns with Solent Trusts HEART values.

Solent GP surgeries and The family Hub are working in partnership together to remove barriers to accessing support and information for families.

The satellite hubs will be running within the surgery alongside baby immunisation clinic

The aim is to capture families who are already at an appointment and offer support and information without having to go to another appointment , another building, another place or at a separate time.
Family Hubs  are key to providing support and early intervention which improves the outcomes for families and children.

As a surgery we are currently reviewing how we engage with our local community and other services, building relationships with our partnership agencies and the charity sector.  Setting up these Satellite Hubs, is an example of how the surgery is work together with other agencies to the benefit our patients and reach out to those who may otherwise be overlooked.

We are also working with the Work and Education Association to provide information and education around access to the NHS and primary care services. Focussing initially on access but moving forward we have asked what information they would like us to  focus on in the ongoing sessions.

This is an ongoing project and we are keen to hear from the local communities about how we can engage further to remove barriers to healthcare. To provide, in equal partnership with them, a model of primary care that truly reflects and represents the neighbourhoods that it serves.

 

Team Spotlight of the Month

Each month, we want to focus on a particular team within Solent GP Surgery and Homeless Healthcare. Hoping to provide information on the roles and responsibilities of each team and a chance to give a more in-depth explanation of what we do here.

In the Spotlight this month: Back Office!

Hi we are Sam, Treena and Tracy and we are the Back Office Team.

We are responsible for the patient referrals and all administration and queries from the Clinicians including letters, emails and anything else the Clinicians require from us. The processing of econsults and booking econsult appointments and dealing with admin queries.  2ww referrals that we monitor and chase until we get an outcome of discharge or a diagnosis.

Administrating safeguarding concerns which includes coding patients records with concerns, completing the safeguarding spreadsheet and completing Section 17 and 47 response templates when they arise from Children’s Services.

Our work comes in many forms, via task, emails, screen messages and econsults.

We are a small team of 3 looking after all the patients within 3 sites and Homeless Healthcare and kept very busy.  Lets just say the days go very quickly.
